I acknowledge that I have received, read and understood the Minor Athlete Abuse Prevention Policy and/or that the Policy has been explained to me or my family.
I further acknowledge and understand that agreeing to comply with the contents of this Policy is a condition of my membership with Wildkit Swimming Organization.
I certify that I am the parent or legal guardian for my child(ren). I hereby give my permission for any
supervisor, coach or other team administrator associated with the Wildkit Swimming Organization to seek and
give appropriate medical attention for our child(ren) in the event of accident, injury, illness. I will be responsible
for any and all costs associated with any necessary medical attention and/or treatment.
I hereby waive, release and forever discharge Wildkit Swimming Organization its directors, officers, agents,
coaches, and employees from all rights and claims for damages, injury, loss to person or property which may be
sustained or occur during participation in Wildkit Swimming Organization activities, whether or not damages or
loss is due to negligence. I hereby acknowledge that my children is (are) physically fit and capable of
participation in all Wildkit Swimming Organization activities.
By registering my child(ren) with the Wildkit Swimming Organization, I agree to participate (or allow my
child(ren) and family members to participate) in the Wildkit Swimming Organization, and hereby
release Wildkit Swimming Organization, its directors, officers, agents, coaches, and employees from liability for
any injury that might occur to myself (or to my child(ren) and family members) while participating in
the Wildkit Swimming Organization program, including travel to and from training sessions, swim meets or other
scheduled team activities.
I agree to indemnify and hold harmless the above mentioned organizations and/or individuals, their agents
and/or employees, against any and all liability for personal injury, including injuries resulting in death to me, my
child(ren) and/or other family members, or damage to my property, the property to my child(ren) and/or other
family members, or both, while I (or my child(ren) or family members) participating in the Wildkit Swimming Organization
Swim Club program.
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other
merchants through this facility that (i) any credit card or bank account draft (ACH Draft) information I supply is
true and complete, (ii) charges incurred by me will be honored by my credit card company or financial
institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes,
fees, and penalties.
I hereby authorize (if online payment is made or autopay information is provided) the Wildkit Swimming Organization
to charge my ACH draft, or credit card account. I understand the withdrawl policy and that I am responsible for fees and
obligations after the withdrawl date whether or not my child attends practices.
Should I dispute a charge through my financial institution this will constitute a breach of contract possibly
resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any
and/or all current and future services.
I give my permission for Wildkit Swimming Organization to use photographs and video of my child(ren) for any lawful purpose,
including but not limited to such purposes as publicity, illustration, advertising, and Web content.
